State disciplines local health care providers

POULSBO — The state Department of Health (www.doh.wa.gov) has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our area.

— In November, the Department of Health suspended the credential of registered nursing assistant Shaveria L. Sneed (NA00122848) for five years. In 2008, Sneed was convicted of theft, and has a previous conviction for a crime of dishonesty.
