State disciplines local health care providers

POULSBO ­— The state Department of Health has taken disciplinary actions or withdrawn charges against the following health care providers in Kitsap.

— In September, the Occupational Therapy Practice Board charged occupational therapist Maryellen Boisen (OT00001900) with unprofessional conduct. Boisen allegedly took a prescription from a patient’s chart, altered the prescription, and tried to have it filled at a local pharmacy. She petitioned to enter drug court on a charge of obtaining a controlled substance by fraud or forged prescription in 2011.
